Our medical office will collect patient records and information for two primary reasons including properly curating a treatment plan and providing you with the best possible medical services from our physicians and staff.
We will not reach out to you via phone, email or mail without your prior consent.
You will always be given the option to opt out of any marketing emails, calls or mail that you are signed to receive. There will be several options for you to do so, including opting out on the online portal or via phone call.
All the data that we have on file for our patients will be accurate, comprehensive and up to date. You may request any and all records we have for you at any time. If you find discrepancies or would like to update your record, you may do so by calling or email our office staff.
All the data collected from our patients is stored in HIPPA compliant computers and programs, to the highest degree of security in accordance with internal security policy and the law.
